Dr. Andrawis and Dr. Gabra have admitting privileges at Fairfax and Fair Oaks Hospitals. Dr. Andrawis is a member of the teaching staff at Fairfax Hospital. Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
Insurance and Payment Information We participate with most insurance health plans. As an added convenience for our patients, we process all insurance claim forms and we also accept charges through checks and credit card. Participating Insurance Companies:
Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients and a copy is available upon request. An acknowledgement form is at the bottom of the notice.
